All Healthy Life EAP clients should review and sign the Statement of Understanding at the beginning of their first EAP visit. This can be kept in the client file and does not need to be sent to us. Follow the instructions on the case report and billing form and please remember that
we must receive billing within 45 days of service, not the date of the last EAP visit. The client satisfaction survey should be given to every Healthy Life EAP client at the time of their last visit.
